DIABETES

Always ask the following;

  1. Diagnosis and events surrounding it
  2. Current and past treatment
  3. Control of BSLs, in the short and long term (very important)
  4. HBA1c level
  5. Symptoms of hyper and hypoglycaemia and awareness thereof/ hospitalisations
  6. Use of glucometer? Use of insulin injections? In what form? Pen?, etc
  7. Medical follow-up – diabetes centre? Specialist?
  8. Dietician? Podiatry and footcare?
  9. Sick day Regime?
  10. Plan for hypoglycaemia?
  11. Vascular risk factors?
  12. Complications including microvascular – eye, kidney, neuropathy, autonomic inc impotence
  13. Complications including macrovascular – AMI, CVA, PVD
  14. Infectious Complications
  15. Family History?
